In Japan, rather than receiving your medicine at the hospital, it is more common to take a piece of paper from the hospital to a pharmacy.
On the day I first went to a hospital, I was asked at the pharmacy, “Do you have a medication record book?” When I stayed silent because I had no idea what it was, the pharmacist showed me a small booklet and told me it is for keeping a record of the medicine you receive.
Medicine you get at a pharmacy comes with a sticker showing the name and the amount. The pharmacy prepares it, so you only have to stick it into the book. On the other hand, medicine you buy for yourself at a shop has no sticker, so you need to write it in yourself.
The book is most useful when you go to several different hospitals. If you know what medicine was given before, you can avoid medicines that should not be taken together. That is possible precisely because the records from every hospital are gathered in a single book.
These days there are apps that let you keep the record on a smartphone, and more people no longer use a paper book. Still, whatever the method, I want to keep my medicine records from becoming scattered.
The correct answer is 2. In paragraph 4, それ points back to the previous sentence — being able to avoid medicines that should not be taken together. Choice 3 mistakes the reason that follows からこそ for the content of それ. Choices 1 and 4 come from paragraphs 1 and 3, not from the paragraph where それ appears.
The correct answer is 4. Paragraph 3 states that medicine received at a pharmacy comes with a sticker showing the name and amount, prepared by the pharmacy, while medicine you buy yourself at a shop has no sticker and must be written in by hand. Choice 1 swaps the two cases, and choices 2 and 3 apply just one of them to both.
The correct answer is 1. The evidence is 一冊に集まっている in paragraph 4 and どんな方法でも、薬の記録がばらばらにならないようにしたい in paragraph 5. Choice 2 takes only the first half of paragraph 5; the writer never says one method is better. Choice 3 overstates with すべて — the book records the medicine you received. Choice 4 is only a scene from paragraph 2.
